This is elementary. It has to happen in order for the insurers to maintain their profits. When you begin to add elective procedures, then people will have procedures that they wouldn't normally obtain because they really don't need it. The most obvious example is plastic surgery. Maybe really ugly people "need" cosmetic surgery, but most people who get cosmetic surgery don't "need" it to live or live well.
